Enrollment Recitals and Agreement

This Education Executed Agreement (the "Agreement") is entered into by and between the Institution named above and the Student named above. By executing this Agreement the parties agree to the terms set forth below, which govern the Student's enrollment in the specified program and the Institution's provision of educational services.

1. Services: The Institution shall provide instruction, curriculum, academic support and reasonable access to facilities as described by the program and applicable handbooks. Instructional schedules, locations and assigned instructors may be modified by the Institution with prior written notice to the Student.

2. Student Obligations: The Student (or Parent/Guardian where Student is a minor) shall comply with academic requirements, attendance policies, conduct standards, and payment obligations set forth in this Agreement and the Institution's published rules. Failure to comply may result in suspension or termination of enrollment.

3. Withdrawal and Refunds: If the Student withdraws from the program, refunds (if any) will be calculated in accordance with the refund schedule agreed at enrollment. The Student acknowledges the following refund schedule: Withdrawal on or before — Refund: ; Withdrawal after — Refund: .

4. Termination for Cause: The Institution may immediately suspend or terminate enrollment for material breach, dishonesty, or conduct that endangers others. Outstanding tuition and fees accrued through the date of termination remain payable.

The Student agrees to abide by the Institution's code of conduct, academic integrity standards, attendance requirements and examination protocols. Academic sanctions for violations may include grade penalties, probation, suspension, or expulsion. The Student consents to reasonable disciplinary procedures as set by the Institution.

Privacy, Records and Consent

The Institution will maintain student records in accordance with institutional policy. The Student (or Parent/Guardian for a minor) authorizes the Institution to maintain and disclose academic records to third parties where necessary for enrollment, financial aid, or safety, and as otherwise permitted by applicable institutional policies. Consent to specific disclosures may be documented separately.

Liability, Indemnity and Dispute Resolution

To the extent permitted by law, the Institution's liability for claims arising out of this Agreement is limited to direct damages up to the amount of tuition paid by the Student. The Student and Institution agree to cooperate in good faith to resolve disputes. The parties agree to pursue mediation prior to initiating formal proceedings; if mediation fails, disputes may be resolved by binding arbitration under mutually agreed rules.

Force Majeure: Neither party is liable for failure or delay in performance due to causes beyond reasonable control, including but not limited to natural disaster, public health emergency, or labor dispute.
