The undersigned agrees to be financially responsible for such emergency medical treatment and releases the Academy, its employees and volunteers from liability for actions taken in good faith to secure treatment. This authorization applies while the student is participating in Academy programs or activities.

Consent to Emergency Treatment: I consent to emergency medical treatment as described above.

Field Trips and Off-Site Activities: The undersigned grants permission for the student to participate in supervised field trips and off-site activities. The Academy will make reasonable efforts to provide notice of such activities and will provide activity-specific consent when required by law or Academy policy.

Tuition Cancellation & Refund Policy

Cancellation and Refunds: Tuition refunds are governed by the Academy's policy. A full refund of tuition paid, less any non-refundable administrative fee, will be issued if written notice of cancellation is received at least fourteen (14) calendar days prior to the program start date. Partial refunds may be granted in exceptional circumstances at the Academy's sole discretion. No refunds are issued after program commencement except as required by law.
