EDUCATION L&E DOCUMENT

This Education L&E Document (the "Agreement") records the express consent, emergency medical authorization, release, indemnity, and acknowledgments required for the Student to participate in school-sponsored programs, activities, and related travel. The undersigned represents that the information provided is complete and accurate and that they have authority to provide the consents herein for the Student identified below.

In consideration of the Student being permitted to participate in the Program, the undersigned consents and agrees as follows:

1. Emergency Medical Authorization: The undersigned authorizes school officials, supervising agents, and medical personnel to obtain and provide emergency medical treatment for the Student if the undersigned cannot be reached. This authorization includes, but is not limited to, emergency medical transportation, hospitalization, anesthesia, and surgical procedures as may be deemed necessary by licensed medical personnel.

I authorize emergency medical treatment as described above.

2. Administration of Medication: If the Student requires medication during Program activities, I authorize designated school personnel to administer such medication in accordance with written instructions I provide. I release the school and its personnel from liability for acts undertaken in good faith pursuant to such instructions.

I authorize administration of prescription and over-the-counter medications as specified in writing.

3. Release, Waiver and Indemnity: The undersigned releases and forever discharges the school, its board, employees, volunteers, and agents from any and all liability, claims, demands, causes of action, or damages arising out of or in connection with the Student’s participation in the Program, except for claims resulting from the gross negligence or willful misconduct of the school. The undersigned agrees to indemnify and hold harmless the school for all liabilities, costs, and expenses (including attorney fees) arising from claims brought by the undersigned or by third parties related to the Student’s participation.

I have read, understand, and accept the release, waiver, and indemnity provisions.

4. Transportation Authorization: I authorize the Student to travel with school personnel or authorized chaperones by school vehicle or contracted transportation as required for Program activities. The school will exercise reasonable care in arranging transportation.

I authorize transportation for the Student as described above.

5. Media and Records Release: I authorize the school to use photographs, video, and other media of the Student taken in the course of Program activities for school publications, promotional materials, and lawful educational purposes. I understand that personally identifiable education records remain subject to applicable privacy protections and that this release pertains only to media and non-sensitive materials.

I consent to use of non-sensitive media as described above.

6. Compliance and Disciplinary Actions: The Student and undersigned agree that Program rules, codes of conduct, and disciplinary procedures apply. The school may remove the Student from Program activities for safety or disciplinary reasons. The undersigned accepts financial responsibility for costs incurred as a result of disciplinary removal or emergency transport.

I acknowledge and accept applicable conduct and discipline policies.

7. Duration and Revocation: This authorization is effective for the Program Dates specified above unless earlier revoked by written notice delivered to school administration. Revocation does not affect actions taken prior to receipt of such notice.

Additional Disclosures

The undersigned certifies that all medical information provided is accurate to the best of their knowledge and agrees to notify the school promptly of any changes. The undersigned further affirms that they have legal authority to enroll the Student and to execute this Agreement.
