Confidentiality, Data Use & Liability

All mentoring communications are confidential between the Mentor and Participant except where disclosure is required by law or where there is a belief of imminent risk of harm. The Mentor and Participant consent to collection of progress data and non-identifying performance measures for program evaluation; personally identifying information will not be disclosed outside the program without explicit written consent. Neither party shall be liable for indirect or consequential damages. This agreement does not create an employment, partnership or fiduciary relationship beyond the mentorship purpose.

Termination and Amendment

Either party may terminate the mentorship for any reason by providing seven (7) calendar days' written notice to the other party. Objectives and timelines may be amended by mutual written agreement; amendments must identify the changed objectives and be signed by both parties to be effective.
