I grant permission for the Provider to photograph, record, and publish images or recordings of the Student for educational and promotional use, subject to applicable privacy protections, unless declined below.

Liability, Insurance and Indemnification

The Provider shall maintain reasonable general liability insurance. To the fullest extent permitted by law, Student and Parent/Guardian release the Provider from claims arising from ordinary negligence. Student and Parent/Guardian agree to indemnify and hold harmless the Provider, its employees and agents from any third-party claims arising from Student misconduct or breach of this Agreement.

Termination; Withdrawal

Provider may terminate this Agreement and remove Student for cause, including safety risks or repeated misconduct. Student/Parent may withdraw by providing written notice to the Provider; refund eligibility is governed by the Refund and Cancellation Policy stated above.

Confidentiality and Records

Provider will maintain student records in accordance with institutional policy and applicable law. Provider may share limited student information with subcontractors engaged to deliver program services; such parties shall be bound by confidentiality obligations.

Representations, Warranties and Acknowledgments

Student/Parent represents that all information provided on this Agreement and accompanying forms is true and complete. Parent/Guardian signing below warrants that they have authority to enroll and bind the Student to the terms of this Agreement if the Student is a minor.

Governing Law; Severability

This Agreement is governed by the laws of the state in which the Provider is located. If any provision is held unenforceable, the remaining provisions shall remain in full force and effect.
