Purpose: The Institution agrees to route the Student to the educational placement, program, or off-site service specified below and to coordinate routing of records and transportation as authorized by the Parent/Guardian or Adult Student. Responsibilities, Liability and Indemnification

Institution Responsibilities: Institution will assign appropriate staff, ensure safe routing and coordination with the transportation provider, maintain required records, and provide reasonable supervision during transit and at the placement site. Institution will carry or require contractor insurance consistent with its standard policies.

Parent/Guardian / Student Responsibilities: Parent/Guardian or Adult Student must ensure timely arrival for pick-up and drop-off, provide accurate medical and emergency information, and notify Institution in writing of any changes impacting routing. Failure to comply may result in suspension of routing services.

Indemnification: To the extent permitted by law, each party agrees to indemnify and hold harmless the other party, its officers, employees and agents from claims arising from the indemnifying party's negligence, willful misconduct, or breach of this Agreement.

Term, Amendment and Termination

Term: This Agreement commences on the Effective Start Date specified above and continues through the Anticipated End Date unless earlier terminated. Either party may terminate this Agreement for material breach upon written notice if the breach is not cured within fourteen (14) calendar days after receipt of notice.

Amendment: Any amendment must be in writing and signed by authorized representatives of Institution and Parent/Guardian or Adult Student.
