  I authorize assignment of benefits or billing to an insurance or third-party payer where applicable.

Client and Provider Obligations

Provider shall deliver Assigned Services with qualified personnel in accordance with the service description above and in compliance with Provider's professional standards. Client agrees to cooperate with scheduling, provide requested records, and ensure timely payment. Client acknowledges obligation to notify Provider promptly of any changes affecting provision of services.

Attendance and Cancellation: Client agrees to provide at least days' notice for cancellation or modification of scheduled services. Provider may assess a cancellation fee if adequate notice is not provided.

Records, Confidentiality, and Release

Provider will maintain educational and treatment records in accordance with Provider's recordkeeping policies. Client authorizes limited disclosure of records to staff and consultative professionals directly involved in providing services and as required by law. Client may authorize release to other third parties by signing a separate release.

Limitation of Liability and Indemnification

Provider's liability for any claim arising out of this Agreement shall be limited to direct damages not to exceed the total fees paid for the Assigned Services described herein. Client agrees to defend, indemnify and hold harmless Provider from claims arising from Client's negligence or willful misconduct in connection with the services, except to the extent caused by Provider's gross negligence or willful misconduct.

Termination and Modification

This Agreement may be modified only by a written amendment signed by both parties. Either party may terminate this Agreement upon written notice to the other party. Termination shall not relieve Client of payment obligations for services rendered through the effective date of termination.

Acknowledgments and Certifications

By signing below, Client certifies that the information provided in this Agreement is true and complete to the best of Client's knowledge, that Client has the authority to enter into this Agreement (or is a parent or legal guardian authorized to so act), and that Client consents to the provision of the Assigned Services under the terms set forth herein.

Client further acknowledges receipt of Provider's service policies and agrees to comply with those policies while services are active.
