EDUCATION SERVICE LEARNING AGREEMENT

Responsibilities

Student responsibilities include compliance with School and Host Organization rules, punctual attendance, completion of assigned tasks, and participation in assessment activities. Student will report immediately any incident or injury to the Host Supervisor and School contact.

Host Organization responsibilities include providing adequate supervision, orientation, safe working conditions, and verification of hours performed. The Host Organization will notify the School promptly of any concerns about student performance or conduct.

School responsibilities include coordinating the placement, communicating academic expectations, and making reasonable efforts to address safety or conduct issues arising during the placement.

Student shall maintain confidentiality of any sensitive information encountered during placement. Unauthorized disclosure of client or organizational information is prohibited and may result in disciplinary action.

Liability, Indemnification, Termination

Liability and indemnification: To the extent permitted by law, the Student and Parent/Guardian agree to hold harmless and indemnify the School and Host Organization from claims arising from the Student's intentional misconduct or gross negligence during the placement. The School and Host Organization will exercise reasonable care and supervision as described in this Agreement.

Termination: Either the School or Host Organization may suspend or terminate the Student's placement for failure to meet conduct or performance requirements or for safety concerns. The School will notify the student (and parent/guardian if minor) promptly of any termination.

Amendment: This Agreement may be amended only in writing signed by the School's authorized representative and the Student or Parent/Guardian.

Governing law: This Agreement will be governed by the laws of the state in which the School is located to the extent permitted by law.

Host Organization Acknowledgment

The Host Organization representative named below acknowledges the placement and agrees to provide supervision and verification of hours as indicated in this Agreement.
