This Education Skill Building Contract (the "Contract") establishes the terms under which the Program Provider will deliver individualized skill-building instruction to the Student identified below. ATTENDANCE, CANCELLATION & REFUND POLICY

Student attendance is essential to achieving stated objectives. The Provider will make reasonable efforts to accommodate excused absences but is not required to provide make-up sessions beyond those expressly agreed. A credit or refund for missed sessions will be provided only as set forth below.

ASSESSMENT, REPORTING & DATA

The Provider will assess Student progress at regular intervals and provide written or electronic reports documenting attainment of objectives, instructional activities performed, and recommended next steps.

CONFIDENTIALITY, RECORDING, AND INTELLECTUAL PROPERTY

The Provider shall maintain the confidentiality of Student records in accordance with applicable educational privacy obligations. The Provider may record sessions only with prior written consent; recordings shall be used solely for instructional or assessment purposes unless otherwise agreed in writing.

I consent to audio/video recording of instructional sessions for assessment and program improvement.    I do not consent to recording.

Intellectual property created by the Student remains the Student's property. Instructional materials provided by the Provider remain the Provider's property unless a written agreement transfers rights.

LIABILITY, HEALTH & EMERGENCY

The Provider will exercise reasonable care in delivery of services. The Student and Parent/Guardian acknowledge and accept that participation involves ordinary risks related to educational activities and, to the extent permitted by law, release the Provider from liability for incidental injuries not caused by gross negligence or willful misconduct.

TERMINATION & AMENDMENTS

Either party may terminate this Contract for material breach upon written notice and a reasonable opportunity to cure. The Provider may terminate for nonpayment in accordance with the payment and refund provisions. Amendments to this Contract must be in writing and signed by both parties.
