EDUCATION SUCCESS ACADEMY AGREEMENT

This Education Success Academy Agreement ("Agreement") is entered into between Education Success Academy, hereinafter "Academy", and the enrolling individual or guardian, hereinafter "Enrollee". Late Payment: A late fee may be assessed for payments received more than days after the due date. Accounts more than days past due may result in disenrollment until the account is current.

Refunds, Withdrawal, and Termination

Withdrawal by the Enrollee must be provided in writing. Refunds, if any, will be prorated based on the Academy's published refund schedule in effect on the date of withdrawal. The Academy reserves the right to terminate enrollment for failure to comply with payment obligations or Academy policies; termination does not relieve the Enrollee of outstanding financial obligations.

Attendance, Conduct, and Academic Standards

The Enrollee agrees to attend scheduled sessions and comply with the Academy's Code of Conduct. Continued enrollment is contingent upon satisfactory academic progress and adherence to behavioral standards. Academic assessments and determinations of satisfactory progress are at the discretion of Academy instructors and administrators.

Health, Emergency Contact, and Medical Authorization

Emergency Medical Authorization: In the event of a medical emergency, the undersigned authorizes Academy staff to obtain emergency medical treatment for the Enrollee and to share relevant health information with medical personnel. The undersigned understands that the Academy will make reasonable efforts to contact the emergency contact listed above.

Media and Records Release

The Enrollee may be photographed or recorded during Academy activities. The Academy may use such media for educational, promotional, or archival purposes. Educational records are maintained by the Academy and released only in accordance with applicable privacy policies and the consent provided below.

Liability, Indemnification, and Dispute Resolution

The Enrollee and guardian release the Academy and its employees from liability for injury or loss except to the extent caused by the Academy's gross negligence or willful misconduct. The Enrollee agrees to indemnify and hold harmless the Academy from claims arising from the Enrollee's actions. Any dispute arising under this Agreement will be governed by the laws applicable to the Academy's principal place of business and resolved through negotiation and, if necessary, binding arbitration.

Miscellaneous

Severability: If any provision of this Agreement is found to be unenforceable, the remainder of the Agreement shall remain in full force and effect. This Agreement constitutes the entire understanding between the parties with respect to the subject matter herein and may be amended only by a written instrument signed by both parties.
