By signing this contract the undersigned agrees to pay tuition according to the payment plan selected. Late payments may incur fees of per occurrence. All unpaid balances may result in exclusion from the program.

Unless otherwise specified in writing, deposit is non-refundable. Partial refunds may be granted only when withdrawal is requested in writing at least days prior to session start, less any non-recoverable costs.

Medical, Emergency & Special Needs

The undersigned authorizes Program Provider and Program staff to administer or arrange for emergency medical treatment for the student if, in the reasonable judgment of staff, such treatment is necessary. The undersigned agrees to be financially responsible for such treatment.

Conduct, Safety & Transportation

Participant is expected to comply with program rules and staff directions. Program Provider reserves the right to suspend or expel any participant whose conduct endangers others or disrupts program operations. No refund will be provided for dismissal due to misconduct.

Permissions, Releases and Liability

In consideration of acceptance into the program, the undersigned releases, indemnifies, and holds harmless Program Provider, its employees, agents, volunteers and affiliates from and against all claims, liabilities, damages, and expenses (including reasonable attorneys' fees) arising out of or related to participation in program activities, except to the extent caused by the gross negligence or willful misconduct of Program Provider.

Data Privacy

Program Provider will collect and maintain personal information required for program administration. Such information will be used for health and safety, reporting, billing, and emergency contact purposes. Provider will take reasonable steps to protect personal data in its possession.

Acknowledgments and Agreement

By signing below, the undersigned acknowledges and agrees that: (1) they have read and understood this contract and accompanying program policies; (2) the statements provided herein are true and complete to the best of their knowledge; (3) they authorize the medical and transportation consents as indicated; and (4) this agreement is binding and governs the rights and obligations of the parties.

Governing Law: This contract shall be governed by and construed in accordance with the laws of the state in which the Program Provider operates, without regard to conflict of law principles.

Entire Agreement: This document, together with any attached enrollment or medical forms signed contemporaneously, constitutes the entire agreement between the parties and supersedes all prior negotiations and understandings. No amendment is effective unless in writing signed by both parties.
