Confidentiality and Records

Supervisor shall maintain the confidentiality of student records and information consistent with applicable law and institutional policy. Termination and Amendment

Either party may terminate this Agreement upon providing written notice to the other party. Required advance notice: days. Supervisor may suspend services immediately where student safety or health is reasonably at risk.

Liability; Indemnification

Supervisor shall exercise reasonable care in the performance of supervision services. Student and Parent/Guardian acknowledge and accept the inherent risks associated with educational activities. To the fullest extent permitted by law, Student or Parent/Guardian agrees to indemnify, defend, and hold harmless Supervisor and its agents from claims arising out of Student's misconduct or failure to follow Supervisor instructions. Nothing in this Agreement limits claims for gross negligence or willful misconduct.
