CONSENT, RELEASE, AND TERMS

Emergency Medical Authorization: In the event of an emergency when the parent/guardian cannot be reached, the undersigned authorizes school personnel to arrange for medical treatment and to transport the student to an appropriate medical facility. The undersigned agrees to be financially responsible for emergency medical service and treatment.

Release and Indemnification: Except to the extent prohibited by law, the undersigned releases and holds harmless the school, its board, employees, agents, and contracted carriers from liability for injury or illness resulting from the provision of transportation services, except for gross negligence or willful misconduct. The undersigned agrees to indemnify the school for claims arising from false or incomplete information provided in this plan.
