Penalties may include suspension of future voucher eligibility and referral for civil or criminal prosecution where warranted.

4. Privacy and Disclosure: Information provided on this form is maintained for administration of the voucher program. Disclosure is limited to program officials, auditors, and others as required by law. By signing, Recipient consents to necessary disclosures for verification and payment.
