EDUCATION WORK-BASED LEARNING EXPERIENCE (WBLE) AGREEMENT

This Work-Based Learning Experience Agreement ("Agreement") establishes the terms under which the School, the Student (and Parent/Guardian if Student is a minor), and the designated Host Site will participate in a supervised, credit-bearing or non-credit work-based learning activity. The parties agree to the provisions below.

LEGAL TERMS, LIABILITY AND TERMINATION

Liability and Insurance: The Host Site is responsible for maintaining appropriate insurance for its operations and shall notify the School in writing of any insurance requirements for placement. The School maintains oversight responsibilities for educational objectives. The Student/Parent understands that participation may involve risk and agrees to comply with safety rules and training provided by the Host Site.

Indemnification: Each party shall indemnify, defend, and hold harmless the other parties from claims arising from that party's negligence or willful misconduct in connection with this WBLE, except to the extent such claims are caused by another party's negligence.

Termination: This Agreement may be terminated by the School or Host Site upon written notice for cause, including but not limited to breach of safety rules, failure to perform assigned duties, or misconduct. The Student may be removed from placement for cause at the discretion of the School or Host Site.

ACADEMIC CREDIT, EVALUATION AND CONFIDENTIALITY

Academic Credit & Evaluation: If academic credit is to be awarded, the School will specify the learning objectives, performance criteria, and evaluation methods. The Host Site will provide regular performance feedback to the School as required by the program.

Confidential Information: The Student agrees to protect confidential information of the Host Site and to comply with any confidentiality or non-disclosure requirements communicated in writing prior to placement.

By signing below, the Student (and Parent/Guardian if applicable) and the School Representative acknowledge that they have read this Agreement, understand the responsibilities and risks, and consent to the Student's participation in the Work-Based Learning Experience under the terms set forth herein.
