The Organizer's cancellation and refund policy is: refunds less any non-recoverable administrative costs if cancellation is received in writing at least 14 days prior to the first day of the workshop. Participant may not copy, distribute, or commercially exploit materials without prior written consent.

Liability, Waiver & Emergency Medical Authorization

By signing this form, Participant (or Parent/Guardian if Participant is a minor) acknowledges and agrees that participation in the workshop involves inherent risks. Participant assumes all risks and expressly releases and holds harmless the Organizer, its officers, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action arising from injury, loss, or damage sustained as a result of participation, except to the extent caused by gross negligence or willful misconduct of the Organizer.

In the event of a medical emergency, the undersigned authorizes the Organizer and its agents to obtain emergency medical treatment for the Participant and to transport the Participant to a medical facility. The undersigned shall be financially responsible for any medical treatment or transport provided.
