9. Notices

All notices required or permitted under this Act shall be sent to the contacts below, or to other addresses subsequently provided in writing.

10. Entire Agreement and Effect

Except as expressly modified by this Act, the Enrollment Agreement and any prior amendments remain in full force and effect. In the event of a conflict between the terms of this Act and the Enrollment Agreement, the terms of this Act shall control with respect to the matters expressly addressed herein.

This Act may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one and the same instrument. Electronic signatures and facsimile or scanned copies of signed counterparts shall have the same force and effect as original signatures.
