All withdrawals must be submitted in writing.

Attendance & Conduct: Regular attendance is expected. The program reserves the right to suspend or dismiss a student for behavior that endangers others, disrupts instruction, or violates the code of conduct. No tuition refund will be provided for dismissal due to misconduct.

Media Release: I authorize the program to use photographs, audio, and video recordings of the student for educational, promotional, and archival purposes. I understand that the student will not be identified by personal address or contact information in publicly distributed media without further consent.

Privacy & Records: The information collected on this form will be used to administer enrollment, maintain student records, and contact guardians in an emergency. I acknowledge the program's right to maintain academic and attendance records in accordance with applicable privacy laws.
