Rights, License, and Use

For good and valuable consideration, the sufficiency of which is acknowledged, I hereby grant to the Institution named below, its successors, licensees and assigns, the irrevocable, worldwide, royalty-free right to record, reproduce, edit, broadcast, distribute, transmit, archive and otherwise use my (or the student’s) name, image, voice, performance, biographical information and likeness ("Materials") in connection with the Production identified above. This grant includes the right to create derivative works, to synchronize said Materials with music or other audio, and to use the Materials in any media now known or hereafter devised.

Compensation: No monetary compensation will be provided unless set forth in a separate written agreement.

Privacy, Confidentiality, and Records

The Institution represents that it will use and disclose personally identifiable information in accordance with applicable educational privacy laws and institutional policy. The Institution may include the Materials in educational broadcasts and archived materials that are accessible to the public or restricted to institutional audiences, as indicated above.

Release, Indemnity and Limitations

I hereby waive any right to inspect or approve the finished production in which the Materials appear. I release and hold harmless the Institution, its employees, agents and assigns from any claims, demands or liability arising out of the use of the Materials, including claims for defamation, invasion of privacy, or infringement of moral rights, except to the extent caused by the Institution's gross negligence or willful misconduct. I agree to indemnify the Institution against any third-party claims arising from inaccurate or incomplete representations made by me in this agreement.
