Photography Consent

By checking the boxes below, the signer authorizes the school to make and use the student's photographic and digital image and audio recordings as described:

Usage, License and Release

I hereby grant to the school, its agents, licensees, successors and assigns a non-exclusive, royalty-free, worldwide, perpetual license to use, reproduce, adapt, publish, distribute, and display the student's image, likeness, voice and biographical information in any media now known or hereafter developed for educational, promotional, archival, or outreach purposes including printed materials, electronic publications, internal and external websites, social media, press releases, and partner publications.

I waive any right to inspect or approve finished photographs or recordings and I release the school and its officers, employees, agents and contractors from any claims, liabilities, damages or causes of action arising out of or connected with the use of the images or recordings, including claims for invasion of privacy, defamation, or misappropriation.

The signer acknowledges that it may not be possible to completely remove images from third party or archival materials once distributed.

I understand that to request withdrawal I must provide a written notice identifying the student and the specific materials to be withdrawn and deliver it to the school's main administrative office. Withdrawal does not retroactively invalidate existing distribution or publications.

Legal Acknowledgments

By signing below, the signer certifies that they are the student (if over 18) or the student's parent or legal guardian and that they have the authority to grant the permissions and releases contained in this Addendum.
