I acknowledge that participation in behind-the-wheel instruction, supervised driving practice, transportation to and from school-sponsored activities, and related training involves inherent risks, including but not limited to property damage, physical injury, or death. I voluntarily assume all such risks on behalf of myself and, if signing as parent or guardian, on behalf of the student and the student's heirs and legal representatives.

In consideration for the student being permitted to participate, I hereby release, waive, discharge and covenant not to sue the school, its board, officers, employees, volunteers, agents, contractors and instructors (collectively, the Released Parties) from any and all liability, claims, demands, actions or causes of action arising out of or relating to any loss, damage or injury, including death, that may be sustained by the student or that the student may cause to others, whether caused by the negligence of the Released Parties or otherwise, arising out of participation in the program or the operation of any vehicle in connection with the program.

I further agree to indemnify, defend and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, costs or expenses (including reasonable attorney fees) arising out of or resulting from my or the student's negligent or intentional acts or omissions while operating a vehicle or participating in program activities.

I represent and warrant that all information provided on this form is true and complete. If personal vehicle use is authorized, I represent that the vehicle is insured with liability coverage at or above the limits required by the program and that I will maintain such insurance for the duration of the program. I agree to notify the program immediately of any material change in insurance or license status.
