The Student will participate in the following program activities (check all that apply):

Participation in environmental fieldwork and outdoor activities involves inherent risks, including but not limited to uneven terrain, inclement weather, insect or animal encounters, exposure to elements, and risks associated with transportation. The Student (or Parent/Guardian on behalf of a minor Student) acknowledges these risks and voluntarily assumes all risks associated with participation.

In consideration of participation, the undersigned hereby releases, waives, and discharges the Institution, its trustees, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating, except to the extent caused by the Institution's gross negligence or willful misconduct. The undersigned agrees to indemnify and hold harmless the Institution from any third-party claims arising from the undersigned's negligent acts or omissions.

Safety, Conduct, and Environmental Compliance

The Student agrees to comply with all safety instructions, to wear required personal protective equipment, and to follow conduct and environmental protection rules established by the Institution and site managers. The Student will not collect or remove flora, fauna, soil, water, or cultural artifacts from any site without prior written authorization or appropriate permits. Violation of safety or environmental rules may result in removal from activities without refund.

Medical Information and Authorization

The undersigned certifies that the medical information provided is accurate to the best of their knowledge and authorizes the Institution to arrange emergency medical treatment if necessary. The Institution will attempt to contact the emergency contact prior to treatment when practicable.

Data, Publications, and Media

The Student understands that activities may produce data, reports, and photographs. The Institution shall have a non-exclusive, royalty-free right to use project data, aggregated analyses, and non-identifying photographs for educational, research, and outreach purposes. The Institution will not use personally identifying information in publicity materials without separate written consent.

Fees, Expenses, and Financial Responsibility

The Student or Parent/Guardian is responsible for fees, transportation, and any personal equipment unless otherwise specified. The Institution will provide notice of any required fees prior to the start of the program.

Term, Termination, and Amendment
