What the Educational Event Outdoor Education Day form is

The Educational Event Outdoor Education Day form is a parent/guardian permission, health release, and activity information form used by schools and organizations to document consent for a supervised off-site outdoor education activity. It combines participant details, emergency contacts, medical disclosures, activity-specific risks, and a signature block to record consent, assumptions of risk, and any required acknowledgments.

Why a clear, complete permission form matters

A properly completed form documents informed consent, reduces liability, ensures medical preparedness, and creates a retrievable record that supports compliance with FERPA for student data and ESIGN/UETA for electronic signatures.

Schools, outdoor education vendors, and youth organizations use this document to obtain parental consent and capture medical and emergency information before outings.

  • School administrators and teachers who organize the trip and maintain records for the district.
