Media Release

Risk, Liability and Medical Authorization

In the event of sudden illness or injury, if the event organizers cannot reach me or another authorized contact, I authorize the event organizers or their designees to obtain and consent to emergency medical treatment for the student. I understand that I am responsible for expenses incurred for such treatment and will make payment through my health insurance or personally as necessary.

The student agrees to comply with all applicable codes of conduct, directions of staff and chaperones, and safety procedures. Failure to comply may result in removal from the activity at the student's expense and without refund of any fees paid.
