This Educational Host Contract ("Agreement") is entered into by and between the Educational Institution and the Host Organization identified below for the purpose of providing a supervised educational placement for the Student named herein. The parties agree to the terms set forth in this Agreement.

Host Organization Responsibilities

The Host Organization agrees to provide a safe and educational environment, appropriate supervision, orientation, and training necessary for the Student to achieve the learning objectives. Host Organization will comply with all applicable laws and regulations, provide required safety equipment, and ensure that duties assigned are consistent with the Student's training level and academic objectives.

Institution Responsibilities

The Educational Institution will select students who meet program prerequisites, provide academic oversight, coordinate placement logistics, and monitor student progress. The Institution retains authority to remove a Student from placement for academic, disciplinary, health, or safety reasons.

Student Responsibilities

The Student shall comply with Host Organization policies, follow directions of the on-site supervisor, maintain confidentiality, attend scheduled hours, and participate in required evaluations. The Student represents that they are physically and mentally able to perform the duties described and will disclose any medical conditions that may affect safe participation.

Background Checks and Screening

The Host Organization may require criminal background checks, child abuse registry checks, drug screenings, or other pre-placement screening as permitted by law. The Host Organization will notify the Institution of screening requirements and will not permit placement of any Student who fails required screenings.

Insurance, Liability, and Indemnification

The Host Organization shall maintain commercial general liability insurance and workers' compensation insurance as required by law for its employees and operations during the placement. To the fullest extent permitted by law, the Host Organization and the Institution agree to indemnify and hold each other harmless from claims arising out of their respective negligence or willful misconduct in connection with this Agreement. Nothing in this Agreement shall be construed to create an employment relationship between the Institution and the Student unless expressly stated in writing.

Confidentiality and Intellectual Property

The Student and Institution shall treat Host Organization proprietary information and trade secrets as confidential. Any intellectual property or work product created by the Student in the course of placement shall be owned by the Host Organization unless otherwise agreed in writing. The Host Organization will notify the Institution and Student of confidentiality obligations and provide necessary agreements where required.

Termination and Suspension

Either party may suspend or terminate the placement for cause, including but not limited to health and safety concerns, breach of this Agreement, failure to meet screening requirements, or unacceptable performance, upon written notice. The Institution may immediately remove a Student from placement where necessary to protect the welfare of the Student or others.

Reporting, Evaluation, and Records

The Host Organization shall report incidents involving injury, safety hazards, or misconduct to the Institution within twenty-four (24) hours. The Host Organization will provide timely evaluations of Student performance as requested by the Institution. Both parties shall maintain records related to the placement in accordance with applicable laws and institutional policy.

Compensation and Academic Credit

Compensation (if any) and the granting of academic credit shall be specified here. Unless otherwise specified, the Student is participating for educational experience and not as an employee of the Host Organization. Any stipend, wage, or credit arrangement must comply with applicable law and be agreed in writing by the parties.

Data Protection and Use

The parties shall use Student personal data solely for purposes of placement administration, monitoring, and evaluation. Each party shall comply with applicable privacy laws and institutional policies governing confidential records.
