REFUND, WITHDRAWAL AND CANCELLATION

Institution's refund and withdrawal policy applies as follows: refunds will be issued only in accordance with the established schedule and any non-refundable fees identified in this Agreement. If Student withdraws prior to the program start date, the refundable portion will be calculated by Institution's standard policy less administrative fees.

ACADEMIC STANDARDS, POLICIES AND CONDUCT

Student agrees to comply with Institution's academic standards, honor code, attendance policy, and any other published rules. Institution reserves the right to impose disciplinary actions, including suspension or dismissal, for breaches of policy, academic dishonesty, or conduct deemed harmful to the school community.

RECORDS, PRIVACY AND CONSENT

Institution maintains student records for academic and administrative purposes. Student/Guardian grants Institution permission to maintain and use records for educational administration, accreditation, and institutional reporting in accordance with Institution policy. Student may designate a third party to receive records by completing an authorized release.

HEALTH, EMERGENCY CONTACTS AND CONSENT

Authorization for Emergency Medical Treatment: I authorize Institution and its agents to obtain emergency medical treatment for the student if required and agree to be responsible for related costs not covered by applicable insurance.

LIABILITY, INDEMNIFICATION AND INSURANCE

To the fullest extent permitted by law, Student/Guardian agrees to indemnify, defend, and hold harmless Institution, its trustees, officers, employees, and agents from and against any claims, liabilities, losses, and expenses arising from Student's conduct or breach of this Agreement. Institution shall not be liable for indirect, special, or consequential damages.

TERMINATION, SUSPENSION AND REMEDIES

Institution may suspend or terminate enrollment for breaches of policy, non-payment, or conduct that jeopardizes safety or the academic environment. Remedies set forth in this Agreement are cumulative and do not limit other available remedies at law or in equity.

DISPUTE RESOLUTION

The parties agree to attempt good faith resolution of disputes arising under this Agreement through direct negotiation. If unresolved, the parties may pursue mediation or other mutually agreed alternative dispute resolution procedures prior to initiating litigation. Governing law shall be the applicable law where Institution is located.

REPRESENTATIONS, WARRANTIES AND ACKNOWLEDGMENT

Student/Guardian represents that the information provided in this Agreement is true and complete, and warrants that they have the authority to enter into this Agreement. Student/Guardian acknowledges receipt of required disclosures and policies where applicable.

CERTIFICATION

By signing below, Student or Parent/Guardian (as appropriate) and an authorized representative of Institution certify that they have read, understand, and agree to be bound by the terms of this Agreement. This Agreement constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes prior oral or written agreements.
