Tuition and Payment Terms

Automatic Payment Authorization: I authorize the Provider to charge scheduled payments as indicated above.

Cancellation, Withdrawal, and Refund Policy

Cancellation and Refunds: The Client may cancel enrollment by delivering written notice to the Provider. If cancellation is received more than fourteen (14) calendar days prior to the program start date, the Client is entitled to a refund of tuition less administrative fees and any non-refundable deposits. If cancellation is received within fourteen (14) days of program start, the Provider may retain a pro rata portion of tuition for services rendered and non-refundable costs. All refunds will be processed within a reasonable administrative period. Failure to attend does not constitute official withdrawal.

Attendance, Make-Up, and Academic Policies

Attendance & Make-up: Regular attendance is required. Missed sessions may be made up only as expressly permitted by the Provider and subject to instructor availability. The Provider will not be responsible for instructional time lost due to Client scheduling conflicts. Excessive absences may result in dismissal without refund as set forth in the Provider's internal policies.

Academic Integrity: Students are expected to adhere to standards of academic honesty. Cheating, plagiarism, or other dishonest conduct may result in disciplinary actions up to dismissal from the program.

Privacy, Records, and Media Release

Confidentiality and Records: The Provider will maintain educational records in a manner consistent with its privacy obligations. The Client authorizes the Provider to collect and retain information necessary for instruction and administrative purposes and to disclose such information to subcontracted instructors and administrative personnel on a need-to-know basis.

Media Release: The Client hereby grants the Provider permission to photograph or record the student during program activities for educational and promotional purposes unless the Client declines by checking the box below. Consent to media use Decline media use

Medical, Emergency, and Special Accommodations

Special Accommodations: The Provider will make reasonable efforts to accommodate documented needs. Requests for accommodations must be submitted in writing and include supporting documentation.

Liability, Intellectual Property, and Termination

Limitation of Liability: To the fullest extent permitted by applicable law, the Provider's liability for any claim arising out of or related to this Agreement shall be limited to direct damages not to exceed the total tuition paid by the Client for the applicable program. The Provider shall not be liable for indirect, incidental, special, or consequential damages.

Intellectual Property: All educational materials, curricula, tests, and instructor-created content provided by the Provider remain the Provider's proprietary materials. The Client is granted a non-transferable, non-exclusive license to use such materials for personal educational purposes only and shall not reproduce or distribute materials without prior written permission.

Termination: The Provider may terminate this Agreement and deny access to services for cause, including but not limited to failure to pay, violation of conduct standards, or threats to safety. Termination for cause may be without refund as specified in Provider policies.

By signing below, the signatory certifies that all information provided on this form is true and complete to the best of their knowledge, and that they have the authority to enter into this Agreement (as the adult student or as the parent/guardian of a minor student).
