Make-up, Rescheduling and Cancellation

Make-up Lessons: Provider allows make-up lesson(s) per package. Make-ups must be requested at least hours in advance and are subject to provider availability.

Cancellation by Provider: Provider may reschedule or cancel due to instructor illness or emergency; in such case Provider will provide notice and offer a make-up or prorated refund where appropriate. Cancellation by Student: Cancellations requested less than hours before a lesson may not be eligible for a make-up or refund.

Student Responsibilities and Conduct

Student/Parent agrees to ensure punctual attendance, preparation for lessons, and respectful conduct. Repeated disruptive behavior may result in suspension of lessons without refund.

Medical, Emergency and Photo Release

Photo/Video Release: I authorize Provider to photograph or record lessons for educational, promotional, or evaluative purposes, subject to Provider's reasonable discretion.

Liability, Insurance and Indemnification

Provider maintains reasonable care in the provision of instruction but is not responsible for personal injury, loss, or damage to personal property except where caused by Provider's gross negligence or willful misconduct. Student/Parent agrees to indemnify and hold Provider harmless from claims arising out of Student's actions, except for Provider's gross negligence or willful misconduct.

Termination and Remedies

Either party may terminate this Agreement for material breach if the breach is not cured within 14 days after written notice. Provider may also terminate for student misconduct or repeated nonpayment. Remedies available to either party shall include recovery of unpaid fees and reasonable costs, in addition to other relief permitted by law.

Dispute Resolution and Governing Law

The parties agree to attempt good faith negotiation of disputes arising under this Agreement. If unresolved, disputes will be governed by the laws of the jurisdiction in which the Provider operates, without regard to conflict of law principles. Either party may pursue equitable relief when necessary to prevent irreparable harm.

Privacy and Records

Provider will maintain records and personal information in a manner consistent with standard educational privacy practices. Provider may use contact information for administrative purposes and to communicate scheduling, billing, and instructional notices.
