I hereby grant to the School and its agents, employees, volunteers and assigns an irrevocable, perpetual, worldwide, royalty-free license to use, reproduce, publish, distribute, display, create derivative works of, and otherwise exploit the media described above that includes the student named in this form, in whole or in part, for educational, promotional, archival and administrative purposes.

I understand and agree that the School may edit, crop or otherwise modify the media and that the School need not obtain further consent from or provide notice to me or the student for such uses. I waive any right to inspect or approve final materials that incorporate the media.

I release and hold harmless the School, its officers, employees and agents from any claims, demands, liabilities or causes of action arising out of the use of the media, including but not limited to claims for invasion of privacy, defamation, or appropriation of likeness, except to the extent such claims arise from willful misconduct or gross negligence by the School.

If the student has authored original work included in the media, I grant the School a non-exclusive, transferable license to use such student work for the purposes described above. I understand that moral rights, if any, are hereby waived to the fullest extent permitted by law.

Revocation: This Release may be revoked in writing by delivering written notice to the administrative office of the School. Any such revocation will not affect uses of media prior to receipt of the revocation and shall not apply to media already published or distributed in any form.
