4. Borrower Responsibilities & Terms

The Borrower acknowledges receipt of the equipment described above and agrees to use the equipment only for bona fide educational purposes consistent with institutional policy. The Borrower accepts responsibility to maintain the equipment in good working order, to follow operating instructions and to return the equipment in the same condition as received, normal wear and tear excepted. The Borrower further agrees to the following:

  1. Not to remove or modify serial numbers, asset tags, or identifying marks.
  2. Not to install unauthorized hardware or software or alter factory settings without prior written approval from the institution's technology department.
  3. To report any malfunction, damage or theft to the institution within 24 hours of discovery by completing an incident report and providing a written statement.
  4. To secure the equipment when not in use and to exercise reasonable care to prevent loss, theft or damage.

The Borrower further authorizes the institution to withhold grades, transcripts, or impose other administrative remedies for unresolved financial obligations arising from damage or loss of institutional property, to the extent permitted by institutional policy and applicable law.
