Cancellation, Refunds, and Liability

Cancellation and Refund Policy: The Provider requires written notice to cancel or reschedule an assessment. Cancellation more than days prior to the assessment date may qualify for a refund less administrative fees. Late cancellations, no-shows, and failure to pay may result in forfeiture of fees.

Limitation of Liability: Except as required by law, Provider's liability for direct damages arising from the assessment services shall be limited to the amount of fees paid for the assessment. Provider is not liable for consequential, incidental, or punitive damages.

Acknowledgement of Policies: I acknowledge that I have read, understand, and agree to the fees, cancellation policy, and limitation of liability as set forth in this Agreement.

Legal Terms

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which the Provider's principal office is located, without regard to conflict of laws principles. Any dispute arising under this Agreement shall be resolved in the competent courts of that jurisdiction.
