Loan of Materials: Provider agrees to loan the specified educational workshop kits to Requesting Organization for the period agreed upon. Requesting Organization accepts custody of the kits on the dates specified and agrees to return all items in the same working condition as received, normal wear excepted.

Condition and Use: Requesting Organization shall ensure kits are used under competent adult supervision. Use in contravention of manufacturer warnings or without appropriate supervision constitutes misuse. Requesting Organization will follow any written safety or handling instructions provided with the kits.

Loss, Damage, and Replacement: Requesting Organization is financially responsible for loss, theft, or damage beyond normal wear. Replacement or repair costs will be invoiced at Provider's standard replacement rates; Requesting Organization agrees to pay within thirty (30) days of invoice.

Indemnification and Insurance: To the extent permitted by law, Requesting Organization agrees to indemnify and hold harmless Provider, its employees and agents from claims arising from the use of the kits. Provider recommends maintaining insurance covering loaned materials and third-party liability when hosting public events.

Cleaning and Return: Kits must be returned clean and complete by the agreed return date. Provider may charge cleaning or sterilization fees if kits are returned in an unsanitary condition or require special cleaning.

Cancellation and Rescheduling: Requesting Organization shall provide Provider with written notice of cancellation or rescheduling as soon as practicable. Provider may assess fees for late cancellations or for costs already incurred.

Compliance: Requesting Organization agrees to comply with all applicable laws, regulations, and school district policies applicable to the workshop and participants.

Certification: By submitting this request and signing below, Requesting Organization certifies that the information provided is true and accurate, that it has authority to bind the requesting entity, and that it accepts responsibility for supervising participants and for the care and return of the kits under the terms above.
