Parent/Guardian will provide copies of immunization and medical records upon request.

Emergency medical authorization: In the event of an emergency where immediate treatment is required and Parent/Guardian cannot be reached, Parent/Guardian authorizes the school to obtain necessary medical care for the student. Parent/Guardian agrees to be financially responsible for such care.

Media release:

Transportation and authorized pick-up: Parent/Guardian authorizes the following additional adults to pick up the student. Provide full names and relationship below.

Records and confidentiality: The school will maintain educational and health records for the student. Parent/Guardian authorizes release of records to authorized educational entities and legal guardians as required for the student's educational placement and health care. The school will comply with applicable laws regarding student records.

Withdrawal and refunds: Notice of withdrawal must be provided in writing. Tuition refunds, if any, will be provided in accordance with school policy; deposits are non-refundable except where expressly required by law or by written school policy.
