Typical signers and their responsibilities

HR Manager

The HR Manager prepares the template, verifies employee eligibility under company policy, confirms performance documentation, and routes the template for approvals; HR ensures the signed record is retained for payroll and audit compliance for the required retention period.

Payroll Administrator

The Payroll Administrator verifies tax withholding instructions, enters the bonus into payroll systems, reconciles payments against approvals, and confirms tax reporting on W-2 forms; payroll holds responsibility for accurate withholding and timely payment.

Common mistakes to avoid when preparing the template

  • Submitting wrong or incomplete taxpayer identification numbers leads to backup withholding, processing delays, and potential IRS penalties; always verify W-9 data in advance.
  • Failing to collect all required electronic approvals or signatures causes payroll rejections and may delay taxable wage reporting for the bonus period.
  • Using ambiguous payment descriptions such as 'year-end payment' without specifying supplemental wage treatment can complicate withholding and state tax reporting.
  • Omitting supporting documentation like performance reviews or policy citations increases audit risk; attach approvals and evidence to the signed template.

Attach supporting evidence
Always attach performance summaries, policy references, or board resolutions to the executed template to substantiate awards during audits or disputes.
