What the Employee Direct Deposit Authorization Form Is

The Employee Direct Deposit Authorization Form is a signed authorization that allows an employer to deposit payroll directly into an employee’s bank account via ACH. It documents the employee’s bank routing number, account number, account type, and written consent to receive wages electronically. Employers keep this authorization to configure payroll, to meet audit and tax recordkeeping obligations, and to resolve deposit exceptions. The form may be submitted electronically or on paper and often requires a voided check or bank verification to confirm account details.

Why this form matters for employers and employees

A clear, signed authorization reduces payroll errors, avoids manual check costs, and creates an audit trail for compliance with wage-payment rules. It documents employee consent for ACH transfers and supports payroll reconciliation, banking verification, and tax reporting.

Why this form matters for employers and employees

Typical users and who completes the form

Employers, payroll teams, HR professionals, and employees commonly use the direct deposit authorization to establish or change electronic payroll deposits.

  • Employees submitting bank account details to receive payroll electronically.
  • Payroll or HR staff collecting authorizations and configuring payroll systems.
  • Third-party payroll providers integrating employee bank instructions into ACH batches.

Use the form whenever an employee enrolls in direct deposit, changes bank details, or when payroll administrators require written authorization for ACH setup.

Common mistakes to avoid when completing the form

  • Entering the routing number with a transposed digit causes failed ACH and delayed pay.
  • Using an old or closed account leads to returned deposits and administrative overhead.
  • Failing to attach a voided check or bank letter lengthens verification and may block activation.
  • Not signing or dating the form, or using initials only, can invalidate employee consent.

Risks and potential consequences of errors

Deposit failure: Returned funds and possible late pay
Backup withholding: 24% rate may apply with incorrect TIN
Reconciliation burden: Extra time and cost to trace and reissue wages
Fraud exposure: Unverified accounts increase impersonation risk
Regulatory fines: I-9 or payroll paperwork penalties possible
