Policy Acknowledgment and Consents

I acknowledge receipt of the Employer's Drug and Alcohol Testing Policy (the "Policy"). I understand that the Policy sets forth the prohibited substances, testing procedures, standards for a positive test, the range of circumstances under which testing may occur (including pre-employment, reasonable suspicion, post-accident, return-to-duty, and random testing), and the disciplinary consequences for violations up to and including termination.

By checking the boxes below I certify that I have received, read, and understand the Policy and I consent to the testing and release provisions described therein.

Authorization to Test and Release Results

I hereby authorize the collection of urine, breath, saliva, blood, hair, or other specimens as required by the Policy for the purpose of drug and alcohol testing. I authorize any laboratory, medical reviewer, healthcare provider, or other agent engaged by the Employer to perform testing and to release test results and related medical information to the Employer's designated representatives, including but not limited to the Medical Review Officer, Human Resources, and the employee's supervisor for the purpose of administering the Policy.

I understand that positive test results or refusals to submit to testing will be handled in accordance with the Policy and applicable law, and that I have the right to request confirmatory testing and to receive information on the available appeals or challenge procedures described in the Policy.

Legal Disclosures

At-Will Employment: I acknowledge that my employment relationship with the Employer is at-will, meaning either I or the Employer may terminate the employment relationship at any time, with or without cause, subject to applicable law. Nothing in this receipt or in the Policy is intended to alter the at-will nature of employment unless expressly stated in a separate written agreement signed by an authorized representative of the Employer.

Equal Opportunity: The Employer is committed to providing equal employment opportunities and does not discriminate based on race, color, religion, sex, national origin, disability, protected veteran status, age, genetic information, or other protected characteristics as defined by applicable law.

Background Checks and Medical Information: I authorize the Employer to obtain background checks and to collect medical information to the extent permitted by law in connection with testing procedures and safety-sensitive positions. I understand that such information will be used in compliance with applicable privacy and employment laws.

Applicant/Employee Certification

I certify that all information provided on this receipt and in any related employment documents is true and complete to the best of my knowledge. I understand that knowingly providing false information or withholding material facts may result in disciplinary action, up to and including termination of employment or withdrawal of any employment offer.

What an Employee Drug Policy Receipt Is and when it’s used

An Employee Drug Policy Receipt is a signed acknowledgement that an employee has received, read, and understood an employer’s drug and substance use policy. The receipt records the policy version, the employee’s name and position, the date of receipt, and a signature or electronic acknowledgement. It can document consent for testing where required, specify testing procedures, and note privacy protections. Employers use the receipt to demonstrate consistent distribution of policy updates, support disciplinary actions when policy violations occur, and maintain a verifiable record for internal audits and regulatory review.

Why documenting receipt matters for compliance and risk control

A clear receipt reduces disputes about whether employees were informed of workplace rules, preserves evidence of notice and consent, and supports lawful enforcement. Electronic acknowledgements are enforceable under the ESIGN Act (15 U.S.C. ch. 96) and UETA where adopted, provided the four-part e‑signature validity test is met.

Why documenting receipt matters for compliance and risk control

Which workplace roles interact with this receipt

Each party plays a role: HR issues and stores the receipt, supervisors enforce rules, and employees provide the signed acknowledgement that completes the record.

  • HR administrators who distribute policies and maintain employee records.
  • Supervisors who verify distribution and note any employee questions or refusals.
  • Employees who must acknowledge receipt and understand testing or disciplinary procedures.

Typical signatories and record owners

HR Manager

HR managers issue the receipt, select retention periods, and ensure records meet company policy and regulatory needs. They coordinate updates when policies change and support compliance reviews during audits or investigations.

Employee

The employee signs or electronically acknowledges the receipt to confirm understanding. Their acknowledgement documents that they received the policy and were informed about testing rules, confidentiality, and consequences for violations.

Essential elements to include on a professional receipt

A complete receipt is concise but includes identifiers, policy reference, and a verifiable signature so the record can be used in audits or disputes.

Employee Details

Full legal name and job title to link the acknowledgement to the correct employment record and avoid ambiguity during enforcement or audits.

Policy Reference

Policy title and version or effective date so reviewers know exactly which policy text the employee acknowledged.

Acknowledgement Text

Short statement the employee received, read, and understood the policy; specify any testing or consent language that applies.

Signature Block

Space for handwritten or electronic signature, printed name, and date; include witness or notary fields only if required by law or company policy.

Testing Consent

If testing consent is part of the policy, note whether acknowledgement constitutes consent and any conditional steps before testing occurs.

Record Metadata

Fields for employer name, document ID, distribution channel, and audit-trail data to support retention and verification.

Required identifying fields for accuracy

Employee name: Full legal name
Position: Job title
Date received: MM/DD/YYYY
Policy version: Title + Effective Date
Signature: Handwritten or e-sign
Record ID: Unique tracking number

Step-by-step: issuing and capturing a signed receipt

Follow a short, repeatable process so each employee acknowledgement is consistent and verifiable.

  • 01
Prepare: Attach current policy and populate employee fields.
  • 02
Send: Deliver via email, secure portal, or in person.
  • 03
Sign: Employee reads and signs or e-signs the receipt.
  • 04
Store: Save signed copy in HR records with audit metadata.

How electronic acknowledgement workflows typically flow

An eSignature workflow captures identity and timing while reducing friction compared with paper distribution.

  • Upload Policy: Load the policy and receipt template to the platform.
  • Add Fields: Place name, date, and signature fields on the receipt.
  • Send Link: Email or generate a secure signing link for the employee.
  • Capture Audit Trail: Platform logs IP, timestamp, and signer events for verification.

Common online settings for secure eAcknowledgements

Configure authentication, retention, and notifications before sending so each receipt meets policy and legal requirements.

Field Configuration
Authentication Email link, SMS code, or stronger MFA
Signature Type Simple e-sign or PKI digital signature
Retention Specify archive period and export options
Notifications Reminders and completion alerts for HR

Delivery channels and integrations that support receipt distribution

Integrations with HR systems and cloud storage reduce manual filing; confirm the platform supports formats like PDF and DOCX and connectors such as Salesforce or Google Workspace.

  • Email delivery: Common; requires secure links and optional passcodes
  • In-person kiosk: Useful for on-boarding; captures local signatures
  • System integrations: Connect to HRIS or storage systems for automatic filing

Timing rules and practical deadlines to follow

Timely distribution and clear update cycles limit disputes and ensure employees are informed of current rules.

At hire:

Provide the policy and obtain receipt on or before the first day of work.

Policy updates:

Re-issue the receipt whenever substantive policy changes occur.

Pre-testing notice:

Provide any required testing notice before administering a test per company or legal rules.

Annual acknowledgement:

Consider yearly re-acknowledgement for safety-sensitive roles.

Record retention:

Follow applicable retention schedules and legal requirements for HR records.

Common mistakes to avoid when preparing receipts

  • Using an outdated policy version or failing to record the version creates ambiguity in enforcement and disputes.
  • Collecting initials without a proper signature or dated acknowledgement can weaken the employer’s evidence of notice.
  • Poor storage practices—scattered files or unsecured email—make retrieval difficult during audits or incidents.
  • Skipping clear consent language when testing is required can lead to employee challenges and procedural delays.

Risks and potential legal consequences of improper documentation

Employment disputes: Claims that policy wasn’t provided or accepted
Regulatory scrutiny: Failure to document can complicate OSHA or DOT inquiries
Privacy exposure: HIPAA concerns for health-related testing data (45 CFR §164.530(j))
Recordkeeping failure: Loss of evidence during audits or litigation
Disciplinary challenges: Difficulty supporting terminations without clear acknowledgement
Operational delay: Inability to proceed with testing or safety actions promptly
