The card must be carried visibly while on employer premises and presented to authorized personnel upon request. The card is issued for the exclusive use of the named employee and must not be transferred, loaned, duplicated, or altered. Unauthorized use of the card is grounds for disciplinary action up to and including termination and civil or criminal prosecution where applicable.

The employer reserves the right to deactivate or reclaim the card at any time. Failure to return the card when required may result in deduction of replacement costs or other lawful remedies.

Photo and Data Consent

By accepting issuance of the identification card, the employee consents to the capture and storage of a photographic image and associated personal data necessary for card issuance and security administration. Such data will be used for identity verification, access control, and internal security investigations, and will be retained in accordance with the employer's records retention policies.

I consent to the capture and use of my photograph and related personal data for card issuance and security purposes.

What an Employee Identification Card Agreement Is

An Employee Identification Card Agreement is a written record that documents the issuance, use, custody, and return of an employer‑issued identification card. It sets out employee responsibilities for physical and electronic card access, acceptable use, lost or stolen card procedures, and return obligations at termination. The agreement may also describe data collected for the card (photo, name, employer ID), retention and privacy protections, and any disciplinary or financial liability for misuse or damage. Employers use this agreement to establish a consistent, auditable policy for identity credentials.

Why a Formal Agreement Matters

A written agreement clarifies obligations, reduces security incidents, and creates an auditable record for compliance and HR actions.

Why a Formal Agreement Matters

Who Typically Completes This Agreement

Employers, HR teams, security administrators, and contractors commonly complete or require the Employee Identification Card Agreement before issuing credentials.

  • Human resources teams handling onboarding, transfers, and terminations with centralized recordkeeping responsibilities.
  • Facilities and security staff issuing physical or logical access credentials and managing access-level changes.
  • Contractors, vendors, and third-party workers who receive temporary cards and must follow site security rules.

Use the agreement to document approvals, access levels, and responsibilities so audits and personnel actions have clear evidentiary support.

  • Approval: Supervisor and security approve access levels.
  • Provision: IT programs badge and updates access control.
  • Record: System logs issuance and stores signed agreement.

  • Integrations: HRIS and access control
  • Authentication: Email, SSO, or MFA
