What the Employee Information Form PIIA Is and when it applies

The Employee Information Form PIIA combines employee personal data collection with a Proprietary Information and Inventions Agreement (PIIA). It records contact and tax data, emergency contacts, employment details, and an employee’s acknowledgement of IP assignment and confidentiality obligations. Employers use it at onboarding and when roles change to document personnel records and protect trade secrets. The form can be completed on paper or electronically; electronic completion must meet ESIGN (15 U.S.C. ch. 96) and applicable state law such as UETA or a state ESRA where adopted.

Why this combined form matters for employers and employees

A unified Employee Information Form PIIA clarifies responsibilities, centralizes critical HR and legal data, and reduces onboarding friction while preserving enforceability of IP and confidentiality terms when properly executed and retained.
