Company-owned uniforms remain the property of the employer and are issued for use in performance of work duties. Employee must maintain issued uniforms in a clean and serviceable condition in accordance with company care standards. Employee is responsible for routine laundering unless otherwise specified in the box below.

Employee must return all company-issued uniforms and identification upon request or upon termination of employment. Employer may require reasonable repair or replacement of damaged or unreturned items. Where permitted by law, employee hereby authorizes deduction from final wages for the unreturned or intentionally damaged items after reasonable notice and opportunity to return items.
