The undersigned authorizes the club to administer first aid and to seek emergency medical treatment as necessary for the safety of the participant.

Assumption of Risk and Release: The undersigned acknowledges that participation in baseball activities involves inherent risks of injury. The undersigned voluntarily assumes all risks and releases, indemnifies, and holds harmless the club, its coaches, volunteers, and affiliated organizations from any and all liability, claims, demands, or causes of action arising out of or related to participation, except for gross negligence or willful misconduct.

Refund & Cancellation Policy: Fees are earned upon provision of services. Refund eligibility is limited and determined by club policy: deposits are non-refundable after roster assignment; prorated refunds for documented medical withdrawal may be granted at the club's discretion. Final determination of refunds is at the sole discretion of the club's executive staff.

Code of Conduct: Participant and guardian agree to comply with the club's code of conduct, including respectful behavior toward staff, officials, and other participants. Repeated violations may result in suspension or dismissal without refund.
