Enrollment Acceptance: By signing this Enrollment Agreement, the enrolling party commits to pay the tuition and fees as set forth above. Acceptance into the chosen session is contingent upon receipt of the deposit and completion of required forms. Failure to timely pay tuition or comply with policies may result in removal from the program without refund.

Cancellation & Refunds: Deposits are non-refundable except where the program is cancelled by the Camp Provider. Refund eligibility for tuition (less deposit) is determined by the camp’s cancellation schedule: full refund if cancellation is received at least 30 days prior to the program start date; 50% refund if received between 29 and 15 days prior; no refund if received within 14 days of program start. Any exceptions must be approved in writing by the Camp Provider.

Release and Liability: In consideration of participation, the enrolling party hereby releases, waives, discharges and covenants not to sue the Camp Provider, its employees, volunteers, and agents from any and all liability for any loss or damage and any claim or demands therefor on account of injury to person or property arising out of or related to participation in camp activities, except to the extent caused by gross negligence or intentional misconduct of the Camp Provider.

Medical Treatment Authorization: In the event of illness or injury requiring medical attention, the enrolling party authorizes the Camp Provider to obtain necessary medical treatment on behalf of the student and agrees to be financially responsible for such treatment.

Photo/Media Release: I grant the Camp Provider permission to use photographs, video, or other media of the student taken during camp activities for promotional, educational, or archival purposes, unless I have indicated an objection by checking the box: .

Code of Conduct: The student agrees to abide by the Camp Provider’s rules and behavioral standards. The Camp Provider reserves the right to dismiss a participant for conduct that endangers others or disrupts the program; dismissal will not entitle the enrolling party to a refund except as may be required under the Cancellation & Refunds clause.

Data & Confidentiality: Information provided on this form will be used for camp administration, emergency contact, and health care purposes. The Camp Provider will maintain reasonable safeguards to protect personal data.

Governing Law & Dispute Resolution: This Agreement is governed by the applicable laws of the jurisdiction in which the Camp Provider operates. Any dispute arising under this Agreement will be resolved by negotiation or, if necessary, by binding arbitration in the Camp Provider’s jurisdiction.
