Fees & Payment Authorization

I acknowledge and agree to the tuition and fee terms below. Tuition is charged per enrolled schedule and is due according to the selected payment plan. Failure to timely pay fees may result in suspension from the program.

Payment Plan: Monthly Weekly Per Session Automatic Bank Draft

I authorize the program to initiate electronic debits to the account listed for tuition payment. I agree to provide 30 days written notice to cancel bank draft authorization.

Program Policies & Acknowledgments

Enrollment is continuous until the program receives written notice of withdrawal. A minimum of 30 days written notice is required to terminate enrollment without liability for the next monthly charge. Fees paid are non-refundable for days missed due to illness, weather, or family vacations unless otherwise stated in a separate written agreement.

Late pick-up fees apply when a child is not picked up by the scheduled program end time. If a child is not collected and no authorized adult can be reached, staff will follow the program's emergency pick-up procedures and may contact child protective authorities if required by law.

The program reserves the right to suspend or terminate participation for failure to comply with behavioral expectations, repeated late payment, or conduct that jeopardizes staff or children. Prior to termination for non-payment or behavioral reasons, the program will provide written notice where practicable.

Liability Release: To the extent permitted by law, I release and agree to indemnify and hold harmless the program, its employees and agents from claims arising from ordinary negligence in the operation of Extended Day activities. This release does not apply to willful misconduct or gross negligence.

Photographs & Media: I consent decline the use of my child's image or likeness in program publications or educational materials. Consent is revocable in writing but may not affect materials already published.
