In consideration of participation in field hockey activities organized by the program, the undersigned acknowledges and agrees to the following terms. Participation in field hockey involves inherent risks, including but not limited to bruises, fractures, concussions, catastrophic injuries or death. The undersigned voluntarily assumes all risks associated with participation.

Liability Release: The undersigned releases, waives, and forever discharges the program, its coaches, agents, volunteers, and representatives from any and all liability, claims, demands, actions, or causes of action for injury, loss, or damage to person or property arising out of or related to participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Indemnification: The undersigned agrees to indemnify and hold harmless the program for any claims brought by third parties arising from the participant's actions or omissions.

Emergency Medical Treatment Authorization: If emergency treatment is necessary and the parent/guardian cannot be reached, I authorize program staff to obtain medical care and treatment for the participant. I agree to be financially responsible for any such treatment.

Refund & Cancellation Policy: Registration fee is non-refundable. Tuition refunds for season withdrawal are subject to prorated credit only at program discretion and based on documented circumstances. The program reserves the right to cancel sessions for insufficient enrollment; in such case, a full refund or credit will be issued.

Participant Responsibility & Code of Conduct: I understand that adherence to safety protocols, equipment rules, and coach directions is mandatory. The program may suspend or dismiss a participant for conduct that jeopardizes safety or interferes with program operation without refund.

I represent that the participant is physically able to engage in field hockey and has not been advised otherwise by a qualified medical professional.
