This Fieldwork Student Contract ("Contract") is entered into by and between the educational institution and the student (collectively "Parties") for the purposes of establishing the terms and conditions governing the student's supervised fieldwork placement.

The student agrees to indemnify and hold harmless the site and the institution for claims arising from the student's intentional misconduct or gross negligence.

Either party may terminate this placement for cause upon written notice. Termination does not relieve the student of obligations accrued prior to termination, including completion of required reports and obligations to confidentiality. Disputes arising under this Contract will be addressed first through informal consultation between the supervisor and academic advisor.
