Emergency Medical Treatment Authorization: If the student requires emergency medical treatment and I cannot be reached, I authorize the supervising staff to obtain medical care for the student, to consent to medical treatment, and to arrange transportation to a medical facility. I understand I am responsible for costs incurred for such treatment.

Photo/Video Release: I grant permission for the institution to record and use photographs, audio, or video of the student for educational and promotional purposes without compensation.    I do NOT grant permission for use of the student’s image.

Transportation: I authorize the student to ride in institution-authorized vehicles or third-party transportation arranged by staff. If I do not consent, I will provide alternate transportation arrangements in writing.

Medication Administration: I authorize staff to administer over-the-counter medication (e.g., acetaminophen, ibuprofen, antacids) in accordance with provided instructions if necessary. I will notify staff in writing of any medications that must not be administered.

Code of Conduct: I understand the student must comply with institution policies, rules, and behavioral expectations during event activities. Violation of policies may result in removal from activities and disciplinary action under applicable student conduct procedures.
