I, the undersigned parent or legal guardian of the student named above, grant permission for my child to participate in the fundraising activity described in this form. 1. I authorize school staff and designated volunteers to supervise my child during the fundraising activity. I understand that volunteers who transport students will comply with applicable licensing, insurance and background check requirements established by the school. I agree to provide any additional driver information upon request.

2. In the event of illness or injury, I authorize the school and its agents to obtain emergency medical care for my child and to take any actions deemed necessary to protect my child's health and safety. I agree to be financially responsible for any emergency medical treatment not covered by the school.

3. I hereby release, waive, discharge and agree to indemnify and hold harmless the school, its governing board, employees, volunteers and agents from any and all claims, liabilities, losses, costs or expenses (including attorney fees) arising out of or in connection with my child's participation in this fundraising activity, except to the extent caused by the wilful or grossly negligent conduct of the school or its employees.

4. I understand and agree that my child and I must comply with all applicable school policies, applicable laws and the instructions of supervising staff and volunteers. Failure to comply may result in withdrawal of participation privileges.

5. I grant permission for my child's image or likeness to be used in school publications, newsletters, or publicity related to the fundraiser, unless I decline below.      

6. I authorize collection, handling and deposit of funds raised by my child for the stated purpose and understand that funds will be handled in accordance with school financial controls. I will not hold the school liable for funds lost as a result of my child's failure to follow collection procedures or for funds knowingly entrusted to another party by my child.
