Rotation / Practicum Title:

Scope of Supervision and Responsibilities

The Preceptor agrees to provide direct supervision, mentoring, and formative feedback consistent with the standards of practice in the field. Supervision will include review of clinical work, discussion of cases, and timely written or verbal feedback. The Preceptor will ensure that the Student has opportunities to meet stated learning objectives and will be available for consultation during scheduled hours.

Learning Objectives and Evaluation

The Preceptor will complete formal evaluations of the Student's performance and submit them to the Academic Program as required. Evaluations will address competence in practical skills, professional conduct, documentation, and adherence to applicable laws and regulations.

What the Graduate Student Preceptor Agreement Covers

A Graduate Student Preceptor Agreement is a written contract that defines responsibilities, supervision levels, learning objectives, schedules, and evaluation criteria between a graduate student and their preceptor or clinical supervisor. It clarifies scope of practice, reporting lines, confidentiality obligations (including HIPAA or FERPA where applicable), liability allocations, and renewal or termination conditions. The agreement typically includes student and preceptor contact information, rotation dates, site details, signature blocks, and record-retention guidance to support academic credit, credentialing, and program audits.

Why a Clear Preceptor Agreement Matters

A written preceptor agreement reduces ambiguity about supervision, protects patient and student privacy, documents learning outcomes, and creates an auditable record for the program and clinical site. It supports compliance with ESIGN/UETA for electronic execution and helps manage risk for institutions, preceptors, and students.

Why a Clear Preceptor Agreement Matters

Who typically completes and signs this agreement

Institutional offices (risk management, legal, or clinical education) often review or retain executed agreements to meet accreditation and audit requirements.

  • Graduate students seeking supervised clinical, research, or field experience tied to academic credit or credentialing.
  • Preceptors or supervisors representing clinical sites, hospitals, clinics, or research labs who will provide direct oversight.
  • Program directors, clinical coordinators, and academic administrators responsible for placement, insurance verification, and compliance.

Step-by-step: completing the preceptor agreement

Follow these steps to prepare, complete, and store an executed agreement so supervision can begin without administrative delays.

  • 01
Prepare Template: Use a program-approved template with required clauses.
  • 02
Populate Fields: Enter names, dates, site, and scope accurately.
  • 03
Obtain Signatures: Get student, preceptor, and institutional signatures.
  • 04
Store Records: Archive signed copy per retention policy.

Typical electronic routing for execution

An electronic workflow speeds completion and preserves an audit trail; consider authentication and consent steps required for your institution.

  • Upload Document: Upload the agreement PDF or DOCX to your e-sign platform.
  • Place Fields: Add signature, date, and initial fields for each signer.
  • Send to Signers: Route to preceptor and student in the required order.
  • Complete & Archive: All signers receive final PDF plus audit log.

Recommended digital workflow settings

Configure these settings to balance signer convenience and institutional risk controls.

Workflow Setting Recommended Configuration
Notifications Email and SMS reminders enabled for signers
Authentication Email link plus optional SMS code for higher assurance
Field Types Signature, date, initials, and checkbox for disclosures
Retention Automatic archive in institutional records system

Technical and file-format considerations

Confirm the platform can export signed PDFs with an audit certificate and integrate with your LMS or student records system for long-term storage.

  • File formats: PDF, DOCX supported
  • Integrations: Sync with Google Workspace or Microsoft 365
  • Security: Encryption and audit logging

Essential elements to include in the agreement

A robust agreement contains specific sections that define responsibilities, supervision structure, and administrative details required by universities and clinical sites.

Parties and Contacts

Identify student, preceptor, sponsoring institution, and clinical site with names, titles, addresses, phone numbers, and email addresses for responsible contacts.

Supervision Duties

Specify the preceptor's supervisory obligations, allowable procedures, delegation limits, on-call expectations, and reporting lines for clinical decision-making.

Schedule and Hours

Define expected hours, shift patterns, required minimum supervised encounters, and procedures for approved absence or make-up time.

Evaluation Process

Describe midpoint and final evaluations, grading criteria, feedback cadence, remediation steps, and who retains evaluation records.

Confidentiality and PHI

State confidentiality obligations, HIPAA compliance requirements, data handling instructions, and any required privacy addenda or BAAs.

Liability and Insurance

Note professional liability insurance requirements, indemnification clauses, and reporting procedures for adverse events or incidents.

Security and compliance checklist

Data Encryption: TLS 1.2/1.3 in transit, AES-256 at rest
Access Controls: Role-based access and SSO
HIPAA BAA: BAA available when required
Audit Trail: Timestamps, IP, and action log
Retention Policy: Configurable retention schedules
Backups: Encrypted backups with disaster recovery

Risks and potential consequences of errors

Confidentiality Breach: Regulatory fines and remediation costs
Scope Violations: Patient-safety incidents and discipline
Missing Signatures: Questionable enforceability
Incorrect Dates: Disputed supervision periods
HIPAA/FERPA Noncompliance: Civil penalties or audits
Liability Exposure: Claims against preceptor or institution

Common preparation errors to avoid

  • Using informal or nickname versions of legal names, which can delay credentialing and create identity-matching problems with institutional records.
  • Leaving scope of supervision vague or open-ended, leading to disputes over permitted procedures and responsibilities during clinical encounters.
  • Failing to secure all required signatures before the rotation, causing programs to pause credit or clinical access until paperwork is complete.
  • Neglecting to include privacy or BAA language when protected health information will be handled, increasing regulatory and reputational risk.

Key dates and scheduling checkpoints

Track these dates to ensure timely approvals, evaluations, and record retention for the supervision period.

Rotation Start:

Documented as MM/DD/YYYY; student gains clinical access

Midpoint Evaluation:

Conduct midway through rotation for formative feedback

Final Evaluation:

Real-world examples of how programs use the agreement

Two representative scenarios show typical clause choices and operational steps for supervision and documentation.

Hospital Clinical Rotation

A university places a student in a hospital for a 12-week rotation with stated supervision ratios and evaluation checkpoints.

  • The preceptor documents weekly feedback in the form.
  • The agreement includes HIPAA language, a BAA with the hospital, and automatic archival of signed PDFs in the student records system to support accreditation.

Research Lab Placement

A graduate student performs supervised lab work involving non-PHI datasets under an IRB-approved protocol.

  • The preceptor signs an IP assignment and data-handling addendum.
  • The agreement clarifies authorship expectations, data ownership, and retention of signed evaluations for thesis review and program records.
