HR AI POLICY TEMPLATE

Effective Date:

Purpose and Scope

This policy sets forth requirements and controls governing the procurement, development, deployment, use, monitoring, and oversight of artificial intelligence, machine learning, and automated decision systems (collectively "AI Systems") by employees, contractors, and agents. The policy applies to all business units and to all individuals who access, use, configure, train, or otherwise interact with AI Systems on behalf of the organization.

Definitions

"AI System" means any software, model, or service that performs tasks by applying algorithms, statistical models, or learned patterns to produce outputs that inform or automate decision-making. "Protected Data" means any personally identifiable information, sensitive personal data, confidential company information, or regulated data.

Permitted Use

Authorized uses of AI Systems include productivity enhancement, data analysis, automation of routine tasks, and customer service augmentation where the system has received requisite approval. All permitted uses require compliance with data protection, intellectual property, and record-keeping requirements specified in this policy.

Prohibited Use

Employees must not use AI Systems to circumvent security controls; to process Protected Data without documented legal basis and approvals; to make final employment, disciplinary, or legal determinations without human review; or to deploy systems that do not meet approved bias, explainability, and accuracy standards.

Model Selection, Approval and Risk Assessment

Prior to procurement, development, or deployment, AI Systems must undergo a formal risk assessment documenting intended purpose, data sources, identified risks (privacy, bias, safety, security), mitigation measures, and designated approvers. The assessment must be retained in the project record.

Data Handling and Privacy

AI Systems that process Protected Data require a documented legal basis and must implement technical and organizational safeguards including, where applicable, de-identification, encryption at rest and in transit, access controls, and retention limitations consistent with data retention schedules.

Transparency, Explainability and Human Oversight

Systems that affect individuals' rights or materially affect business outcomes must include documentation describing inputs, limitations, and a human oversight plan. Automated decisions that materially affect employees or customers require human review and an avenue for appeal or correction.

Security, Change Control and Vendor Management

AI Systems are subject to security testing, change control, and vendor due diligence. Third-party models and services must be assessed for data handling, model provenance, and update controls. All model changes that materially alter outputs require re-assessment and re-approval.

Monitoring, Logging and Audit

Use of AI Systems must include logging of requests, inputs, outputs, and model versions sufficient to support monitoring, audit, and incident investigation. Periodic performance checks against defined metrics (accuracy, fairness, drift) are required.

Incident Reporting and Remediation

Suspected AI incidents, including data exposure, biased outputs, or safety failures, must be reported immediately to Information Security and HR using the organization's incident reporting process. Remediation plans must prioritize cessation of harm, notification where required, root cause analysis, and corrective actions.

Disciplinary Measures and Compliance

Violations of this policy may lead to disciplinary action up to and including termination, as well as civil or criminal liability where applicable. Compliance with this policy is mandatory for all staff and contractors.

Policy Review

This policy will be reviewed periodically and updated as needed to reflect changes in technology, legal requirements, or business practices. Amendments shall be communicated to all affected employees.

What the HR AI Policy Template Is

The HR AI Policy Template is a structured document employers use to define rules, roles, and controls for using artificial intelligence in human resources processes. It provides standardized sections for scope, model governance, data handling, employee rights, training requirements, and approval workflows so organizations can document consistent practices across recruitment, performance management, benefits administration, and workforce analytics while aligning with applicable privacy and employment laws.

Why a Formal HR AI Policy Matters

A formal policy clarifies responsibilities, reduces legal and operational risk, and supports consistent decision-making when AI affects hiring, evaluation, or employee data. It helps satisfy regulatory expectations for transparency, consent, and recordkeeping under ESIGN/UETA, HIPAA (when applicable), and state privacy laws.

Why a Formal HR AI Policy Matters

Who Typically Prepares and Uses This Template

The completed policy is distributed to managers, data teams, and employees and used during procurements, audits, and training.

  • HR leadership coordinating policy language, governance, and employee notifications.
  • Legal or compliance teams reviewing regulatory implications and contractual language.
  • IT or data science groups documenting technical controls and model monitoring.

Essential Sections to Include in the HR AI Policy Template

A professional template organizes governance, technical controls, and operational steps so reviewers and implementers can find obligations and responsibilities quickly.

Purpose & Scope

Define the policy's objectives, covered HR processes (recruiting, performance, benefits), and which organizational units and personnel the policy governs.

Definitions

Provide clear definitions for 'AI', 'automated decision', 'model owner', 'data subject', and other terms to avoid ambiguity in interpretation and enforcement.

Data Privacy Controls

Specify permitted data sources, retention limits, anonymization requirements, and any special handling for sensitive or protected data such as PHI or student records.

Model Governance

Describe model selection, validation, bias testing, versioning, performance metrics, and scheduled revalidation procedures tied to deployment risk levels.

Roles & Responsibilities

List accountable parties (HR, legal, IT, model owner), approval authorities, escalation paths, and required documentation for decisions and exceptions.

Training & Oversight

Include employee training cadence, audit schedules, incident response steps, and monitoring indicators to detect degradation or disparate impact.

Step-by-Step: Completing the HR AI Policy Template

Follow these sequential steps to draft, review, and publish a policy that aligns with organizational controls and legal obligations.

  • 01
Draft: Populate sections with factual system and process descriptions.
  • 02
Internal Review: Route to legal, privacy, and IT for comments.
  • 03
Approval: Obtain signatures from designated approvers.
  • 04
Publish: Distribute to employees and record the final version.

Configure an Online Review and Approval Workflow

Set up digital routing rules and authentication to mirror the policy approval chain and capture an audit trail.

Field Configuration
Signature Authentication Email with optional SMS code or SSO identity verification
Approval Order Sequential routing: HR → Legal → IT → Executive
Version Control Require new version number on edits and preserve prior versions
Audit Capture Enable timestamps, IP, and signer attribution for traceability

Where to Send the Completed Policy and Related Records

Determine authoritative destinations for executed policies, implementation guides, and monitoring logs to maintain a single source of truth.

  • HR Document Repository: Store the signed policy in the HR records system.
  • Legal Archive: Retain an executed copy in the legal document library.
  • Model Registry: Link covered AI systems to the policy in the model inventory.
  • Audit Logs: Export monitoring logs into the compliance archive.

Technical and Integration Considerations

Validate that the chosen platform records an audit trail, supports conditional fields, and can export signed documents for long-term retention.

  • Supported Formats: PDF, DOCX, and searchable PDF
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Authentication: SSO, SMS codes, advanced signer auth

Common Mistakes to Avoid When Preparing the Policy

  • Leaving the scope vague, which allows inconsistent or unauthorized AI use across teams and systems.
  • Failing to identify data sensitivity or PHI and omitting required HIPAA safeguards or BAAs.
  • Skipping model validation steps and bias testing, increasing the likelihood of discriminatory outcomes or regulatory exposure.
  • Not establishing a clear approval and change control process, which complicates audits and incident response.
