What the HR Relocation Information Form Is

The HR Relocation Information Form collects employee relocation details that HR, payroll, and relocation vendors need to coordinate moves. Typical entries include employee contact and emergency information, current and new addresses, planned move and reporting dates, reimbursement elections, estimated moving costs, tax-withholding preferences, immigration or licensing notes, and required approvals. The form creates a single source of truth for approvals and expense processing and supports retention, auditability, and secure electronic records under applicable e-signature laws.

Why a Standardized Form Matters for Relocation

A consistent HR Relocation Information Form reduces errors, ensures correct tax treatment of reimbursements, centralizes approvals, and preserves a clear audit trail that supports compliance with ESIGN and state electronic transaction laws.

Why a Standardized Form Matters for Relocation

Clear role separation reduces rework and speeds processing from request through payment.

  • HR generalists and relocation coordinators who collect and verify employee move data and approvals.
  • Payroll or finance teams that process reimbursements, tax withholding adjustments, and backup-withholding checks.
  • Employees being relocated who provide personal details, preferred payment method, and timing for the move.

Cost Estimates

Itemize estimated moving costs, anticipated reimbursements, payee preferences, anticipated advances, and cost-center coding to ensure accounting and budget approval alignment.

Tax Treatment

Record whether reimbursements are taxable, any gross-up instructions, backup withholding status, and whether a relocation allowance is subject to payroll withholding.

Approvals

Designate approvers, capture signature blocks or e-sign fields for employee, HR representative, and finance approver, and include dates for each authorization.

Supporting Attachments

Provide space for receipts, offers from relocation vendors, lease or home-sale documentation, immigration paperwork, and other scanned documents to support reimbursement.
