What the HR Testing Program Form Is

The HR Testing Program Form documents an employer’s procedure for pre-employment, periodic, post-incident, or return-to-duty testing of employees. It records employee identity, test type, collection date and time, consent or authorization, specimen tracking, and results disposition instructions. The form also captures chain-of-custody details and required acknowledgments so testing events are auditable and defensible. Employers use the form to align testing with workplace policies, regulatory obligations, and privacy rules governing medical and biometric data.

Why a Standardized Form Matters for HR Testing

A standardized HR Testing Program Form ensures consistent collection of consent, identity, and chain-of-custody data, reduces administrative errors, and supports legal defensibility under applicable privacy and employment laws.

Core sections to include in a professional form

A complete HR Testing Program Form groups identity, consent, collection, lab routing, results, and retention instructions so each testing event is auditable and consistent with policy.

Identity

Fields for full legal name, employee ID, date of birth, and verifier initials to prevent misidentification and ensure lab specimens match the correct individual.

Consent

Explicit consent checkbox and signature area documenting the employee’s agreement to the test and acknowledgment of the testing policy and consequences.

Collection Details

Collection date/time, collector name, collection site, specimen ID, and tamper-evident seal information to support chain-of-custody integrity.

Chain of Custody

Sequential transfer entries with timestamps and signatures for each handler, plus tracking numbers for shipments to the testing laboratory.

Results and Actions

Designated area for lab results, reviewer name, date, and employer-required next steps such as return-to-duty or disciplinary procedures.

Privacy & Retention

Notice of how medical/test results will be stored, who may access them, and retention timelines consistent with HIPAA and company policy.

Essential data elements to capture

Date of Birth: MM/DD/YYYY format
Test Type: Specific test name

Common preparation and processing mistakes to avoid

  • Incomplete identity fields or inconsistent employee names that break chain-of-custody and delay lab matching.
  • Missing or unclear consent language that creates legal exposure for privacy or employment disputes.
  • Failure to record accurate timestamps for collection and transfer, undermining specimen integrity or regulatory compliance.
  • Using unsecured channels to transmit results, increasing the risk of unauthorized disclosure of protected health information.

Potential liabilities and compliance risks

HIPAA Exposure: Civil penalties possible
Discrimination Claims: Employment litigation risk
OSHA Violations: Recordkeeping fines possible
State Penalties: Per-state fines vary
Invalid Results: Testing delays or retests
Data Breach: Notification obligations
